About forms
Use SmartFlow form to record information electronically. For example, use a form to record new patient information or record a patient owner's consent before a procedure.

- Select Forms, then select New form to make a form from the template
Form templates and PINs
A practice manager can configure a form template to use a PIN. As a result, staff must enter a PIN before they can complete a form that uses the applicable form template as its source. For example, a practice manager can configure a form template PIN for a surgery consent form. Thus, a client cannot complete the form unless a staff member enters their PIN. And a staff member can do a check of the consent form before they complete it.
Form buttons
A form has three buttons. This table gives each button and its function:
| Button | Function |

| Save | If you select Save, SmartFlow saves the form but does not complete the form. Select Save when it is necessary to complete the form at a different time. |
| Finalize | Select Finalize to complete the form. After you complete the form, you cannot change the form data. |
| Cancel | Select Cancel if you do not want to keep changes that you made to the form. |
