Settings of the Create Form dialog box

You can use the Create Form dialog box to configure a form. It is important that you know the function of the settings of the dialog box.

This table shows each setting and its function:
SettingFunction
Create from existing formUse Create from exists form to make a copy of a form that you made before. If you select a form, SmartFlow automatically makes the settings of the Create Form dialog box the same as the form that you selected.
TitleUse Title to enter the name of the form.
Note: SmartFlow automatically enters the same text in the Internal Name box.
Internal NameUse Internal Name to show the name that SmartFlow shows to staff.
Submit Button TextUse Submit Button Text to record the name of the button that clients select to complete the form.
Show PhotoUse Show Photo to configure if the form shows the patient image.
Show on Main ScreenUse Show on Main Screen to control if SmartFlow shows the form on the whiteboard or the dashboard.

To configure SmartFlow to show the form on the whiteboard, set Show on Main Screen to YES.

To configure SmartFlow to show the form on the dashboard, set Show on Main Screen to NO.

Retain report after dischargeUse Retain report after discharge to control if SmartFlow automatically downloads the form to your device after you discharge the patient.
Request passcodeUse Request Passcode to control if a person must enter a passcode before SmartFlow shows the form. For example, if you want only staff to complete a form, set Request passcode to Yes. As a result, clients cannot use the form.
