Make a form

SmartFlow has electronic forms that staff can use to quickly and easily get client information. A practice manager can configure forms as necessary.

To configure a form, do this procedure.
  1. Select the user account button.

  2. Select SETTINGS.

  3. In the TEMPLATES AND FORMS section, select Forms.

    SmartFlow shows the Forms screen.
  4. Select NEW FORM.

    SmartFlow shows the Create Form dialog box.
  5. In the Title:* box, enter the name of the form.

  6. Make the other settings as necessary.
  7. Select Done.

SmartFlow has the form.
