About forms

You can use forms to record information electronically. For example, you can record new patient information or record a patient owner's consent before a procedure.

Practice managers can make as many forms as necessary. For example, they can make:
  • Admission forms
  • Surgery consent forms
  • Euthanasia forms
After a practice manager configures the form, they must sync the form to your organization's mobile devices. After they sync the form, you can:
  • Use the plus button of the left sidebar of a whiteboard to make a form
  • Use the dashboard of a flowsheet to make a form
Note: You cannot use the SmartFlow web app to make a form. You must use the mobile app.

A practice manager can configure a form so that staff must enter a PIN before they can complete the form. For example, if you have a surgery consent form, the practice manager can configure the form to ask for a PIN. As a result, a staff member must do a check of the consent form before they can complete the form. And the customer cannot complete the form without a staff member entering their PIN.

Buttons of a form

A form has three buttons. It is important that you know the function of each button:
ButtonFunction
SaveIf you select Save, SmartFlow saves the form but does not complete the form. For example, if you must use the iPad to complete a different procedure, you can select Save and complete the form later.
FinalizeSelect Finalize to complete the form. After you complete the form, you cannot change the data of the form.
SubmitIf you select Submit, SmartFlow saves the form but does not complete the form. For example, if you must use iPad to complete a different procedure, you can select Submit and complete the form later.
Note: A practice manager can change the name of the Submit button. If a practice manager does not change the name of the button, SmartFlow shows Submit.

About the sync of forms information to different PIMS

If your SmartFlow site is integrated with a practice information management system (PIMS), your PIMS can sync client information and patient information to SmartFlow. As a result, SmartFlow can automatically add the information to forms. This table shows the which PIMS can sync which data:
