About forms

Use SmartFlow form to record information electronically. For example, use a form to record new patient information or record a patient owner's consent before a procedure.

Each form uses a form template as its source. To make a form template, a practice manager uses the SmartFlow web app. Examples of form templates include:
  • Admission forms
  • Surgery consent forms
  • Euthanasia forms
After a practice manager makes a form template, they must sync the template to your organization's iPads. After they sync the template, you can:
  • Use the plus button in the left sidebar of a whiteboard to make a form from the template
  • Select Forms, then select New form to make a form from the template
Note: You can only use the SmartFlow iPad app to make a form from a form template.

Form templates and PINs

A practice manager can configure a form template to use a PIN. As a result, staff must enter a PIN before they can complete a form that uses the applicable form template as its source. For example, a practice manager can configure a form template PIN for a surgery consent form. Thus, a client cannot complete the form unless a staff member enters their PIN. And a staff member can do a check of the consent form before they complete it.

Form buttons

A form has three buttons. This table gives each button and its function:

ButtonFunction
SaveIf you select Save, SmartFlow saves the form but does not complete the form. Select Save when it is necessary to complete the form at a different time.
FinalizeSelect Finalize to complete the form. After you complete the form, you cannot change the form data.
CancelSelect Cancel if you do not want to keep changes that you made to the form.
